Contract awarded for delivery of sets of materials and accessories necessary for cataract surgery using the phacoemulsification method, including the lease of a phacoemulsification and posterior vitrectomy device, as well as implants and tools used in ophthalmology for a period of 24 months - case no. z/88/pn/24 1. the subject of the order is the successive deliveries by the contractor to the ordering party of sets of materials and accessories necessary for cataract surgery using the phacoemulsification method, together with the lease of a phacoemulsification and posterior vitrectomy device, as well as implants and tools used in ophthalmology for a period of 24 months, of the type and in the quantities specified in the price form constituting appendix no. 2 to the draft terms and conditions, on the principles specified in appendix no. 4 to the draft terms and conditions. 2. detailed description of the subject of the order by tasks: the subject of the agreement is the successive deliveries by the contractor of: ? task no. 1 – deliveries of sets of materials and accessories necessary for cataract surgery, using the phacoemulsification method, together with the lease of a phacoemulsification and posterior vitrectomy device ? task no. 2 – deliveries of lenses ? task no. 3 – deliveries of orbital implants ? task no. 4 – deliveries of conformers ? task no. 5 – deliveries of hemostatic sponges ? task no. 6 – deliveries of ophthalmic knives ? task no. 7 – deliveries of disposable cannulas ? task no. 8 – deliveries of assortment for testing visual potentials for the erg device; 3. the contractor may submit an offer for one, several or all lots. during the examination and evaluation of offers, the ordering party will consider each task separately. 4. the contractor should offer the subject of the order in accordance with the parameters specified by the ordering party. wherever the ordering party has not explicitly specified a dimension or other parameter, this is the responsibility of the contractor. 5. if the description of the subject of the order contains references to standards, technical assessments, technical specifications and reference systems referred to in art. 101 sec. 1 item 2 and sec. 3 of the public procurement law, the ordering party, in accordance with art. 101 sec. 4 of the public procurement law, allows the use of solutions equivalent to those described. each time a standard, technical assessment, technical specification or technical reference system is indicated in this sqm, it should be assumed that the phrase "or equivalent" has been used in relation to them. 6. if the ordering party indicates a trademark, patent or origin, source or special process that characterises the products in the description of the subject of the order – it should be assumed that the indicated trademarks, patents, origin etc. define the minimum parameters indicated in annex no. 2 to the sqm, which constitute the criteria used to assess equivalence. each time a trademark, patent or origin, source or special process that characterises products is indicated in this sws, it should be assumed that the phrase "or equivalent" has been used in relation to them (legal basis art. 99 sec. 5 and 6 of the public procurement law). 7. the ordering party has specified in appendix no. 2 to the sws the quality requirements relating to at least the main elements that make up the subject of the order. the contractor is obliged to provide in appendix no. 2 to the sws: the manufacturer and the trade name or catalogue number. value of the result: winner selection date : date of conclusion of the contract :12/12/2024 lot-0002:title: task no. 2 – lens supply lot-0002:description: 1. the subject of the order is the successive deliveries by the contractor to the ordering party of sets of materials and accessories necessary for cataract surgery using the phacoemulsification method, together with the lease of a phacoemulsification and posterior vitrectomy apparatus, as well as implants and tools used in ophthalmology for a period of 24 months of the type and in the quantities specified in the price form constituting annex no. 2 to the technical specifications, on the principles specified in annex no. 4 to the technical specifications. 2. detailed description of the subject of the order by tasks: the subject of the contract is the successive deliveries by the contractor for task no. 2 – supplies of lenses. 3. the ordering party informs that in the proceedings in question conducted in the form of an open tender, it envisages the possibility of applying the procedure referred to in art. 139 sec. 1 of the public procurement law. the ordering party, using the aforementioned procedure, will first examine and evaluate the offers, and then subjectively qualify the contractor whose offer was assessed the highest in terms of the lack of grounds for exclusion and meeting the conditions for participation in the proceedings. .delivery of sets of materials and accessories necessary for cataract surgery using the phacoemulsification method, including the lease of a phacoemulsification and posterior vitrectomy device, as well as implants and tools used in ophthalmology for a period of 24 months - case no. z/88/pn/24
